Fleet & heavy-equipment paperwork

The paperwork a fleet manager actually keeps in the binder.

Hour meters, service intervals, DOT walkarounds — the three things a shop foreman asks a fleet about on Monday morning. Most PM-log quarters come back half-closed, the DOT-drift across mixed equipment (excavators, skid-steers, mini-loaders) is impossible to audit, and the binder gets tossed in the truck when the next job rolls. The Gridpin fleet paperwork is one sign-off form per machine, front-to-back, in the order the shop foreman writes it — with US Letter and A4 layouts and a single commercial-use license.

EQ-04 is the anchor: per-shift pre-trip equipment check — WL-01 rides alongside it for the one-page-per-shift PM log, CC-01 for the GC / sub construction contract, and CSB-01 bundles all twenty-five working-super forms (EQ-04, WL-01, CC-01, the low-voltage install sign-off, the fiber pull record, the RFI tracker, and the rest) under one license. Drafted on a real clipboard by a real shop foreman. Not a generic daily log, not an invoice template with a fleet sticker on it.

What a fleet kit carries

Four sections a fleet shop actually fills out on a real PM-log quarter.

The kit follows the order a shop foreman writes it on a clipboard — pre-shift unit and operator, PM log with service intervals, DOT walkaround with defects, then the daily-log and work-order cross-reference. No duplicate fields, no “create your own” rows, no AI-generated trade-buzzword filler.

Unit + operator + shift

A working-foreman pre-shift block the shop reads at 6:30 a.m.

Machine ID, hour meter, operator, shift start — the small strip a shop foreman needs to read before the trucks roll. Same row order a working foreman writes it on a clipboard, not the language a DOT over-the-road pre-trip uses.

PM log + service interval

Per-row service intervals (250 / 500 / 1000 h) with the last-service and next-due stamps.

Service interval, last service date, next-due stamp, mechanic initials — the four rows that close the PM-log quarter. The fields the shop foreman and the mechanic agree on at the parts counter, not the language an equipment vendor datasheet reads.

DOT walkaround + defects

Tires, lights, leaks, attachments, safety gear — pass / fail / n-a with a defect note line.

Pass / fail / n-a toggle on every row, a defect note line, a re-check stamp so the booked-out operator sees their own sign-off when they throw the binder open on Monday. Same layout the DOT inspector expects, adapted for excavators, skid-steers, and mini-loaders.

Daily log + RFI / work-order

A one-page-per-shift daily log row strip with a short work-order block riding beneath the walkaround sign-off.

Crew + weather + delays on the front, work-order number and short description on the back. The cross-reference a PM asks for at the Monday owner meeting, on one page, no separate kit.

Working-pro seal of approval

What working pros say after a season of filling out the binder.

Field-language quotes from the people who actually carry the kit on the truck, between the truck and the gate, or between rounds — not a marketing-pull-quote deck, not an after-the-fact testimonial swap.

Shop foreman · 6-truck fleet
DOT pre-trips were written for over-the-road trucks and we run excavators and skid-steers. The EQ-04 walks the machine the way the morning walkaround actually goes — tires, fluids, hydraulics, attachment, safety gear, operator sign-off. The check ride lives, the audit holds.

Pre-trip is the thing my shop reads at 06:45 before I take the keys. EQ-04 has hydraulic, engine oil, coolant, DEF, rear strobe — the rows I actually look at. Defect line is short enough I fill it before I throw the binder in the cab.

Fleet manager · mixed equipment
The PM-log quarter used to close half-empty because nobody could find the last-service stamp on a mini-loader that changed hands twice. EQ-04 + WL-01 ties the shift sign-off to the work-order block, so the mechanic sees the open defect before the machine is back for service.
